Medicare Advantage PPO and Private Fee for Service (PFFS) plans may be out of network. Confirmation of your "In-Network" status can be made by calling your insurance.

Medicare Advantage HMO plans (e.g., SecureHorizons, MD Care Healthplan, and other Medicare Advantage HMO plans through networks such as Empire Physicians Medical Group and Desert Oasis Medical Group) cannot be accepted due to HMO requirement that HMO members must see their HMO primary care physician.

At this time, Eisenhower 365 cannot accept HMO programs such as California Care, SCAN Healthplan or other Medicare Advantage or commercial HMO plans through networks such as Empire Physicians Medical Group or Desert Oasis Medical Group due to HMO requirements that HMO members must see their HMO primary care physician.
